    Athlete Registration

    Please complete the Cibola HS Athlete Registration Form.

     

    You must complete the Life of an Athlete course and Concussion course prior to completing this form.  You will get a code upon completion of those two courses-insert the code when you get to that portion of the registration

     

    Please also review and prepare the Athletics Physical Forms
