The State of New Mexico now requires all 9-11th graders to have 2 doses of Varicella (chicken pox) vaccine in order to enroll in school.  Please check your records and if your child needs the second dose you can schedule an appointment with your doctor or the Health Department for the vaccination for no charge.  The nearest office is at 7704 Second St NW in Albuquerque. Call for appointment at 505-897-5700.  The nurses can answer questions pertaining to any vaccinations.  Please bring your immunization record to registration.  If the immunizations are not up to date, the student will not be allowed to register and receive a schedule.  If you get a yearly exemption from the State, please bring the State approved exemption form. Please be sure to read all instructions to the New Mexico Immunization Exemption form as it has been recently changed.
